Within a primary health framework proactively provide a community nurse consultancy, expert advice and coordination service to acute hospital providers to support the identification of complex patients and/or patients with a long length of stay to enable care needs to be met in the most appropriate environment.

The Role:

With­in a pri­ma­ry health frame­work proac­tive­ly pro­vide a com­mu­ni­ty nurse con­sul­tan­cy, expert advice and coor­di­na­tion ser­vice to acute hos­pi­tal providers to sup­port the iden­ti­fi­ca­tion of com­plex patients and/​or patients with a long length of stay to enable care needs to be met in the most appro­pri­ate environment.

Sup­port the iden­ti­fi­ca­tion and facil­i­tate the trans­fer of care for patients with less com­plex care needs who present to emer­gency depart­ments and/​or admit­ted to short stay units whose care needs could be safe­ly man­aged with­in the com­mu­ni­ty setting.

Under­take a broad con­sul­ta­tive role by build­ing and main­tain­ing the knowl­edge with­in acute hos­pi­tal providers about the knowl­edge, skills, and exper­tise of com­mu­ni­ty nurs­ing and the types of care that can be man­aged in the com­mu­ni­ty by com­mu­ni­ty nurses.

Main­tain a com­pre­hen­sive care net­work, build­ing and main­tain­ing links with providers in the pri­ma­ry health set­ting includ­ing com­mu­ni­ty ser­vice providers, Gen­er­al Prac­ti­tion­ers, Res­i­den­tial Aged Care Facil­i­ties and the Rur­al Hospitals.

Mersey-Lev­en Com­mu­ni­ty Health Nurs­ing and Home Care Services

Mersey-Lev­en Com­mu­ni­ty Health Nurs­ing and Home Care Ser­vice pro­vides nurs­ing care at the com­mu­ni­ty clin­ics in Devon­port and Ulver­stone, and Home care ser­vices. The team con­sists of Nurse Unit Man­ag­er, Clin­i­cal Nurse Con­sul­tants, RNs, ENs and ancil­lary staff includ­ing atten­dants, admin and home help/​personal carers.

Nurs­ing care includes med­ica­tion assis­tance, wound care – includ­ing com­plex VAC dress­ings, pal­lia­tive care, catheter changes and IV antibiotics.

Home care ser­vice includes hygiene & domes­tic assis­tance; and cov­ers areas includ­ing but not lim­it­ed to Devon­port, Ulver­stone, Latrobe, Port Sorell, Shear­wa­ter and Sheffield includ­ing rur­al dis­tricts of Gunns Plains, Niet­ta, Gowrie Park and Bak­ers Beach.

Oth­er ser­vices pro­vid­ed are –

  • CoNECs (Com­mu­ni­ty Nurs­ing Enhanced Care Ser­vice) — Free short-term care at Devon­port clin­ic for clients requir­ing IV Abs & wound care includ­ing burns care)
  • HITH pro­gramme (Hos­pi­tal in the Home) — Clients are man­aged in their home & remain under the care of a hos­pi­tal-based team.

Details of Appointment

Per­ma­nent full time day work posi­tion work­ing 76 hours per fort­night com­menc­ing Novem­ber 2022.

Salary: $102, 080 — $ 108, 528 per annum + 10.5% super­an­nu­a­tion + access to salary packaging

You’ll have access to salary pack­age a range of ben­e­fits includ­ing liv­ing expens­es up to $9,010 + $2,650 in meal enter­tain­ment each FBT year + oth­er ben­e­fits if eligible.

In addi­tion, the fol­low­ing allowances cal­cu­lat­ed on the salaried incre­men­tal point may be available:

  • Post Grad­u­ate Allowance of up to 7.5% based on qual­i­fi­ca­tion type
  • Pro­fes­sion­al Devel­op­ment Allowance of up to $750 per annum
  • Salary range is in accor­dance with Nurs­es and Mid­wives (Tas­man­ian State Ser­vice) 2019


Suc­cess­ful appli­cants will be required to meet the essen­tial criteria:

  1. Reg­is­tered with the Nurs­ing and Mid­wifery Board of Aus­tralia as a Reg­is­tered Nurse

  • Holds or is work­ing towards a rel­e­vant post grad­u­ate ter­tiary qualification
  • Cur­rent Driver’s Licence
