The four pil­lars that under­pin the Nurse Nav­i­ga­tor role, coor­di­na­tion of con­sumer cen­tred care; Cre­at­ing part­ner­ships; Improv­ing con­sumer out­comes and Facil­i­tate sys­tems improve­ment. Work­ing with­in the four pil­lars, this role aims to facil­i­tate a safe and effi­cient con­sumer jour­ney, through the reduc­tion of ser­vice frag­men­ta­tion, edu­cat­ing and empow­er­ing indi­vid­u­als and coor­di­nat­ing con­sumer care.

The role is intend­ed to tran­scend access mul­ti­ple spe­cial­i­ties, sup­port nurse-led ser­vice mod­els and aug­ment exist­ing mod­els of care with­in a facil­i­ty or clin­i­cal area.

Salary Infor­ma­tion (Nurse Grade 7):

Total remu­ner­a­tion val­ue is com­prised of;

• A salary between $73.81 — $80.14 p.h. (includes 23% casu­al loading)
• 10% employ­er super­an­nu­a­tion contribution

Vac­cine Pre­ventable Dis­eases (VPD) Requirements:

It is a con­di­tion of employ­ment for this role for the employ­ee to be, and remain, vac­ci­nat­ed against vac­cine pre­ventable dis­eases as list­ed on the Role Description.
