This posi­tion offers a career chang­ing oppor­tu­ni­ty for an expe­ri­enced Pre­ventable Chron­ic Con­di­tions Edu­ca­tor to work with a dynam­ic team focussed on improv­ing the health of indi­vid­u­als and their fam­i­lies in a cul­tur­al­ly unique envi­ron­ment. The role pro­vides spe­cial­ist sup­port to pri­ma­ry health cen­tre staff to imple­ment key areas of the North­ern Ter­ri­to­ry Chron­ic Con­di­tions Pre­ven­tion and Man­age­ment Strat­e­gy, with spe­cif­ic focus on improv­ing out­comes in ear­ly detec­tion and man­age­ment of pre­ventable chron­ic conditions.

Expe­ri­ence dri­ving a man­u­al vehi­cle and a cur­rent NT man­u­al dri­ver’s licence and a will­ing­ness and abil­i­ty to trav­el by road or light air­craft to remote com­mu­ni­ties for nego­ti­at­ed peri­ods is required to ful­fil the inher­ent respon­si­bil­i­ties of this role”.

You will be a reg­is­tered nurse hold­ing a cur­rent cer­tifi­cate of reg­is­tra­tion with the Nurs­ing and Mid­wifery Board of Australia.
