The oppor­tu­ni­ty

Sil­ver Chain are cur­rent­ly look­ing for a Case Coor­di­na­tor to join their team based at their Albany Ser­vice Centre. 

This is a full time, fixed term con­tract for up to 5 months. Pool car can be pro­vid­ed for work pur­pos­es. If you are an inter­nal appli­cant, this role is offered as a sec­ond­ment role.

Key respon­si­bil­i­ties

Become part of a ded­i­cat­ed and pas­sion­ate team with­in Coun­try Health Ser­vices. You will be account­able for the safe deliv­ery of qual­i­ty evi­denced based clin­i­cal prac­tice to the clients in your area, and respon­si­ble for the com­pe­ten­cy and per­for­mance of clin­i­cal staff in your team. 

About you

• Cur­rent Reg­is­tra­tion with the AHPRA
• Demon­strat­ed expe­ri­ence in lead­ing a team and coor­di­nat­ing resources to achieve outcomes.
• Demon­strat­ed deci­sion mak­ing and prob­lem-solv­ing skills to inno­v­a­tive­ly man­age the pro­vi­sion of qual­i­ty client care.
• Well-devel­oped com­mu­ni­ca­tion and inter­per­son­al skills, with capac­i­ty to nego­ti­ate, prob­lem solve and influ­ence indi­vid­u­als and organisations
• Evi­dence of a rel­e­vant Post Grad­u­ate qualification
• Demon­strat­ed abil­i­ty to work autonomous­ly with a team focus
• A cur­rent Aus­tralian driver’s licence 

Under the Aged Care act 1997, suc­cess­ful can­di­dates will be required to pro­vide a sat­is­fac­to­ry Nation­al Police Clear­ance pri­or to commencement.
