We are seek­ing a Nurse Clin­i­cal Coor­di­na­tor (Psy­chogeri­atric) to join our Aged Care Unit. In this role you will work as part of a mul­ti­dis­ci­pli­nary team to pro­vide com­mu­ni­ty based sup­port to eli­gi­ble peo­ple with age-relat­ed men­tal health issues. The ser­vice pro­vides ear­ly inter­ven­tion, assess­ment and care coor­di­na­tion for old­er per­sons with men­tal health dis­or­ders, as well as edu­ca­tion and advice to ser­vice providers across the Cen­tral Aus­tralian and Bark­ly regions.

You must have exten­sive expe­ri­ence work­ing with old­er peo­ple with chron­ic dis­ease, demen­tia and/​or men­tal health con­cerns, cur­rent AHPRA reg­is­tra­tion and an abil­i­ty to trav­el to remote regions.
