Nurse Practitioner - Complex Care/Rheumatic Heart Disease

An opportunity has become available within the East Arnhem Population Primary Health Care team to work as part of a multidisciplinary health team and engaging closely with other service providers.

The position will be responsible for overseeing and managing the Rheumatic Heart Disease –RHD- program on the 4 communities in the Groote Eylandt Archipelago, requiring close collaboration with the RHD unit in Darwin as well as agencies such as NT Cardiac.
The NP will work closely with the Remote Area Clinic staff including Remote Area Nurses- RANs, Clinic Managers and GPs to provide a proactive and preventative approach to service delivery in this High Priority Program
