Tell me more… The Clin­i­cal Nurse Con­sul­tant – Dia­betes Edu­ca­tor will pro­vide clin­i­cal lead­er­ship and sup­port to the Tor­res and Cape Hos­pi­tal and Health Ser­vice (TCHHS), Cook­town MPHS, for Dia­betes Man­age­ment of clients with­in the Cook­town area. This role presents an oppor­tu­ni­ty to par­tic­i­pate in inter­ven­tive strate­gies to mit­i­gate risks involved with chron­ic dis­ease man­age­ment for patients and pro­vide for improved health out­comes using qual­i­ty stan­dards with­in agreed time­frames and resources. 

As a CNC Dia­betes Edu­ca­tor work­ing in Tor­res and Cape HHS, your con­tri­bu­tion in help­ing treat, reha­bil­i­tate and improve the lives of patients is huge!
