Tell me more… The Clinical Nurse Consultant – Diabetes Educator will provide clinical leadership and support to the Torres and Cape Hospital and Health Service (TCHHS), Cooktown MPHS, for Diabetes Management of clients within the Cooktown area. This role presents an opportunity to participate in interventive strategies to mitigate risks involved with chronic disease management for patients and provide for improved health outcomes using quality standards within agreed timeframes and resources.
As a CNC Diabetes Educator working in Torres and Cape HHS, your contribution in helping treat, rehabilitate and improve the lives of patients is huge!
