Katherine West Health Board (KWHB) is currently looking for a Discharge Care Coordinator to join their team for an opportunity in the Katherine West region in the NT. This position will be well suited to someone with a recognised qualification in Aboriginal Health Practitioner or Registered Nurse with no restrictions on practice.

Estab­lished in 1998, Kather­ine West Health Board (KWHB) is an Abo­rig­i­nal Com­mu­ni­ty Con­trolled organ­i­sa­tion that pro­vides a range of clin­i­cal, pre­ven­ta­tive and pub­lic health ser­vices to clients in the Kather­ine West Region of the North­ern Ter­ri­to­ry. With four major health care cen­tres and a range of satel­lite cen­tres, KWHB ser­vices an area over two times the size of Tas­ma­nia (162,000km²), help­ing to improve the health and well­be­ing of some of the most remote com­mu­ni­ties in Australia.

Posi­tion Description

Kather­ine West Health Board (KWHB) is cur­rent­ly look­ing for a Dis­charge Care Coor­di­na­tor to join their team for an oppor­tu­ni­ty in the Kather­ine West region in the NT..This posi­tion will be well suit­ed to some­one with a recog­nised qual­i­fi­ca­tion in Abo­rig­i­nal Health Prac­ti­tion­er or Reg­is­tered Nurse with no restric­tions on prac­tice. With expe­ri­ence in coor­di­nat­ing care, case con­fer­enc­ing and dis­charge plan­ning, prefer­ably in a remote or Indige­nous health context.

This role ensures seam­less coor­di­na­tion of care and ser­vices dur­ing the dis­charge process from ter­tiary and inter­state hos­pi­tals, fos­ter­ing con­ti­nu­ity of care and opti­mal client
out­comes.

Your respon­si­bil­i­ties will include (but are not lim­it­ed to):

  • Com­mu­ni­cate proac­tive­ly and effec­tive­ly with hos­pi­tal staff includ­ing doc­tors, nurses,and social work­ers, to gath­er rel­e­vant patient infor­ma­tion and dis­charge plans.
  • Act as an advo­cate for the clien­t’s needs and pref­er­ences dur­ing the dis­charge plan­ning process.
  • Ensure trav­el for clients return­ing to their home com­mu­ni­ties, whether by air, road, or oth­er means, is appro­pri­ate and includes nec­es­sary equip­ment such as wheel­chairs or escorts.
  • Arrange for nec­es­sary med­ical equip­ment and sup­plies to be avail­able upon the clien­t’s return home.
  • Refer patients to rel­e­vant sup­port ser­vices, such as men­tal health ser­vices, NDIS and social care ser­vices as required.
  • Par­tic­i­pate and con­tribute to con­tin­u­ous qual­i­ty improve­ment with­in KWHB.
