Tell me more…The Clin­i­cal Nurse Con­sul­tant (CNC) posi­tions are per­ma­nent and offers the unique oppor­tu­ni­ty to work at Pri­ma­ry Health Care Cen­tres (PHCC) with­in the Tor­res Strait. The CNC will assist in the devel­op­ment and pro­vi­sion of Acute and Pri­ma­ry Health Care Ser­vices, men­tor­ing and sup­port­ing the pri­ma­ry health care clin­i­cians through­out North­ern TCHHS. As a Clin­i­cal Nurse Con­sul­tant, you are the clin­i­cal lead in the pri­ma­ry health care set­ting, work­ing in part­ner­ship with the Indige­nous Health Work­ers, and pro­vid­ing pri­ma­ry health care and chron­ic dis­ease man­age­ment as well as emer­gency care. The Out­er Island PHCCs work with­in the Tor­res Mod­el of Care with oper­a­tional man­age­ment by the Indige­nous health workforce.
