We support staff working in aged care through education, information, and online resources.
The CRANAplus After Hours Aged Care Project, funded by the Northern Queensland Primary Health Network, aims to upskill Residential Aged Care Facility (RACF) staff and other service providers to deliver comprehensive aged care, thereby assisting in the management of after-hours service needs.
This collaborative project has strengthened existing relationships and referral pathways, facilitated new partnerships and improved links and networking between the RACF and their local GPs, hospitals, pharmacists, after-hours services, and local palliative care communities.
Together we are committed to improving resident care so aged Australians can acquire the care they need at the right time and in the right place. Delivered to six specified sites in Far North Queensland this innovative project continues to focus on the unique needs of the aged care workforce through face-to-face education as well as online eLearning resources.
