The CRANAplus After Hours Aged Care Project, fund­ed by the North­ern Queens­land Pri­ma­ry Health Net­work, aims to upskill Res­i­den­tial Aged Care Facil­i­ty (RACF) staff and oth­er ser­vice providers to deliv­er com­pre­hen­sive aged care, there­by assist­ing in the man­age­ment of after-hours ser­vice needs. 

This col­lab­o­ra­tive project has strength­ened exist­ing rela­tion­ships and refer­ral path­ways, facil­i­tat­ed new part­ner­ships and improved links and net­work­ing between the RACF and their local GPs, hos­pi­tals, phar­ma­cists, after-hours ser­vices, and local pal­lia­tive care communities. 

Togeth­er we are com­mit­ted to improv­ing res­i­dent care so aged Aus­tralians can acquire the care they need at the right time and in the right place. Deliv­ered to six spec­i­fied sites in Far North Queens­land this inno­v­a­tive project con­tin­ues to focus on the unique needs of the aged care work­force through face-to-face edu­ca­tion as well as online eLearn­ing resources.
