As the Nurse Clinical Specialist – Chronic Conditions, you will provide specialist clinical care, with a focus on improving outcomes through illness prevention, early detection, and management of preventable chronic conditions in the context of the ASCC

Prison Health pro­vides clin­i­cal ser­vices to adults sen­tenced and resid­ing at Alice Springs Cor­rec­tion­al Cen­tre (ASCC), with­in a mul­ti­dis­ci­pli­nary ser­vice deliv­ery health team framework. 

As the Nurse Clin­i­cal Spe­cial­ist – Chron­ic Con­di­tions, you will pro­vide spe­cial­ist clin­i­cal care, with a focus on improv­ing out­comes through ill­ness pre­ven­tion, ear­ly detec­tion, and man­age­ment of pre­ventable chron­ic con­di­tions in the con­text of the ASCC. You will over­see client Adult Health checks, imple­ment client care plans, lead and coor­di­nate client spe­cial­ist con­sul­ta­tions through tele­health video­con­fer­enc­ing ser­vices, and pro­vide sup­port­ive edu­ca­tion to staff. 

To be suc­cess­ful, you will:

  • Have cur­rent reg­is­tra­tion with the Nurs­ing and Mid­wifery Board of Aus­tralia or eli­gi­ble for the same. 
  • Have high­ly devel­oped clin­i­cal knowl­edge and skills in a rel­e­vant area of nursing/​midwifery relat­ing to this vacan­cy, sup­port­ing well devel­oped clin­i­cal deci­sion-mak­ing and prob­lem solv­ing to achieve best prac­tice health­care outcomes. 
  • Demon­strate and pro­mote a learn­ing envi­ron­ment through clin­i­cal sup­port, instruc­tion, and feed­back includ­ing men­tor­ing stu­dents, grad­u­ates and less expe­ri­enced staff across the mul­ti­dis­ci­pli­nary team. 
  • Have well-devel­oped knowl­edge and under­stand­ing of the social and cul­tur­al issues, influ­ences and sen­si­tiv­i­ties that influ­ence health care pro­vi­sion for clients and families.
